Foliotek “Digital Portfolio” Assignment – Due December 15

In order to receive certification to teach in Georgia, you have to prove that you have the knowledge, skills, and disposition to teach. The agricultural education faculty needs to examine artifacts that demonstrate this knowledge, skill, and disposition. Click here for the list of artifacts need to be loaded into Foliotek by December 15. The artifacts will be individually scored using the rubric at the end of this document. The Agricultural Education faculty use these artifacts to determine, in part, whether or not to recommend you for a teaching certificate. “Proficient” or higher scores are preferred for each standard. Directions for accessing Foliotek are here.


Verification of Placement Form – Due January 3.

Click here for the Verification of Placement form. Complete this form and return to Dr. Barry Croom, Department of Agricultural Leadership, Education, and Communication, 260 Rainwater Road, Tifton, GA 31793, due no later than the first day of student teaching. You may scan and return this form to Dr. Croom as an email attachment at dbcroom@uga.edu. A legibly handwritten copy of this form is acceptable. This form confirms beyond doubt that you are student teaching at the correct school, and provides university faculty with information about the courses you will be teaching.

Teaching Calendar – Due January 3.

Prior to beginning student teaching, student teachers will consult with cooperating teachers and develop a teaching calendar of topics to be taught by the intern. There is no form to submit for this assignment. Design your calendar using your own preferences. Your teaching calendar should include:

  • A schedule of when you will take over each course.
  • A plan for the topics (unit) covered each week in each of the courses you will teach.
  • FFA activities and other activities that you will attend or that have direct bearing on your student teaching experience.
  • School holidays.

Pre–Student Teaching Experiences – Due January 3.

Include the following items completed prior to the student teaching experience: Teaching Philosophy Statement (one page), Behavior Management Plan (one page), & Résumé (no page limit). There is no specific form or format for these assignments, other than to format them in a professional manner.

Student Teaching Experiences – Due January 10.

Include the completed list of student teaching experiences with documentation from the cooperating teacher. This form is not complete unless all blanks are filled in.

Weekly Reports – Due Sundays by 11:55 PM.

Student teachers will keep a weekly account of where hours are spent during after school and on weekends. Reports should be submitted to the Georgia Ag Ed website, just as every agriculture teacher in Georgia submits their monthly reports.  Monthly reports will be due each Friday at midnight. The directions for this assignment are in the student teaching handbook.


Reflection Journal – Due Fridays by 11:55 PM.

Student teachers will keep a weekly journal of their experiences during student teaching. Journals will be submitted to the university supervisor by midnight each Friday and a copy of the submitted entry retained in this section of the portfolio.

SAE Visits – Due last day of student teaching.

A description of all SAE visits made during student teaching should be included. A portion of those visits should be made with the cooperating teacher and other visits should be made by the student teacher without the cooperating teacher present. For all SAE visits the SAE Visit Record form should be completed and entered into the portfolio.

Mock Interview – Due last day of student teaching.

Schedule and prepare for a mock interview with the administrator who would be responsible for interviewing an agricultural education teacher at your cooperating center. Take your résumé with you and have the interviewer use and complete the Mock Interview Feedback form.

Lesson Plans & Teaching Materials – Lesson plans for the next week at due on Fridays at 11:55 PM.

Lesson plans are required for all classes. Use your preferred cloud storage provider (DropBox, Google Drive, OneDrive, etc.) to organize and store your lesson plans. Provide access to your cooperating teacher (CT) and to your university supervisor (US). Your CT and US must be able to easily access your lesson plans at any time. Remember, edTPA materials should not be stored online.


Weekly Evaluation Forms & University Supervisor Evaluations – Due last day of student teaching.

Include copies of all the weekly evaluation forms and copies of the evaluation forms completed when your university supervisor visits.

Cooperating Teacher Final Evaluation Form – Due last day of student teaching.

Using the Cooperating Teacher Evaluation Form, provide us with feedback about the performance of your cooperating teacher. If you worked with more than one teacher, provide a separate form for each teacher.
